Contoh Prosedur Seleksi Obat Baru untuk Formularium Rumah Sakit

1. Komite Farmasi dan Terapi bekerja sama dengan Instalasi Farmasi melakukan pendataan untuk proses seleksi terhadap obat
yang akan digunakan di Rumah Sakit.
2. Komite Farmasi dan Terapi mempertimbangkan obat yang akan digunakan dengan memperhatikan faktor sebagai berikut :

5 Permintaan obat baru susulan standarisasi, dokter pemohon mengajukan usulan obat baru melalui formulir permohonan obat
baru non formularium dengan memberi keterangan mengenai :
a.Spesifikasi obat (nama dagang, dosis, nama generik, pabrik)
b.Alasan permintaan obat tersebut
c.Perkiraan jumlah dan lama obat digunakan.
6 Komite Farmasi dan Terapi mempertimbangkan usulan dengan memperhatikan faktor-faktor yang telah ditetapkan untuk proses
seleksi.
7 Bila telah disetujui, Unit Purchasing akan membuatkan item master di sistem komputer.
8 Unit Purchasing akan memesan barang sesuai dengan standar prosedur yang berlaku.
