PELAPORAN DAN ANALISIS INSIDEN KESELAMATAN PASIEN

Rumah sakit menetapkan sistem pelaporan insiden keselamatan pasien baik internal maupun eksternal.
Maksud dan Tujuan
Insiden keselamatan pasien yang selanjutnya disebut insiden ialah setiap kejadian yang tidak disengaja dan kondisi yang mengakibatkan atau berpotensi mengakibatkan cedera yang dapat dicegah pada pasien, terdiri atas kejadian tidak diharapkan, kejadian nyaris cedera, kejadian tidak cedera, ejadian potensial cedera, dan kejadian sentinel.
Pelaporan insiden keselamatan pasien yang selanjutnya disebut pelaporan insiden ialah suatu sistem untuk mendokumentasikan laporan insiden keselamatan pasien, analisis, dan solusi untuk pembelajaran. Sistem pelaporan diharapkan dapat mendorong individu di dalam rumah sakit untuk peduli akan bahaya atau potensi

bahaya yang dapat terjadi pada pasien. Pelaporan juga penting digunakan untuk memonitor upaya pencegahan kesalahan (error) sehingga dapat mendorong dilakukan investigasi. Di sisi lain, pelaporan akan menjadi awal proses pembelajaran untuk mencegah kejadian yang sama terulang kembali.
Rumah sakit perlu menetapkan sistem pelaporan insiden antara lain meliputi

  1. kebijakan;
  2.  alur pelaporan,
  3. formulir pelaporan;
  4. prosedur pelaporan;
  5. insiden yang harus dilaporkan, yaitu kejadian yang sudah terjadi, potensial terjadi, ataupun yang nyaris terjadi;
  6. siapa saja yang membuat laporan;
  7. batas waktu pelaporan.

Selain laporan internal, rumah sakit juga wajib membuat laporan ke Komite Nasional Keselamatan Pasien sesuai dengan peraturan perundangan-undangan dan dijamin keamanannya, bersifat rahasia, anonim (tanpa identitas), serta tidak mudah diakses oleh yang tidak berhak.
Dengan pelaporan ekternal tersebut maka berarti rumah sakit telah ikut berpartisipasi dalam penyediaan data nasional angka insiden keselamatan pasien, pembelajaran untuk meningkatkan mutu pelayanan dan keselamatan pasien bagi rumah sakit lain, dan ditetapkannya langkah-langkah praktis keselamatan pasien untuk rumah sakit di Indonesia.

  • Rumah sakit menetapkan regulasi sistem pelaporan insiden internal dan eksternal sesuai dengan peraturan perundang-undangan yang meliputi butir 1 sampai dengan 7) pada maksud dan tujuan.(R)
  • Ada bukti unit kerja telah melaporkan insiden keselamatan pasien.(D,W);
  • Rumah saki tmengintegrasikan pelaporan kejadian dan pengukuran mutu agar solusi serta perbaikan yang dilakukan terintegrasi.(D,W)
  • Ada bukti rumah sakit telah melaporan insiden keselamatan pasien setiap 6 bulan kepada representasi pemilik dan bila ada kejadian sentinel telah dilaporkan di setiap kejadian.
  • Ada bukti rumah sakit telah melaporkan insiden keselamatan pasien kepada Komite Nasional Keselamatan Pasien sesuai dengan peraturan perundang- undangan.(D,W)
